A screenshot of the device:

On the left is my one-click generated version, on the right is the one in the schematic. The lapis blocks are an example of where you could place them.
I made a module that allows you to teleport up to 16 blocks up by simply placing a lapis lazuli block on each level. You go to a higher level by jumping, and a lower level by sneaking. You do need to walk at least a tiny bit while sneaking, as you probably expected. I tried working on a way to not need the player to walk, but I don't think that would be a better way. It can handle multiple teleports within 16 blocks, it always goes to the closest one first. I have a really big schematic included which shows really well how it works, I'm not sure if the armor stand works correctly, since it looks a bit weird in MCEdit, but I promise you I teleported it up .3 blocks! I also made a one-click version of it, but I'm not sure where you normally want it to be summoned in, so it summons it in at the default place, a little bit above the command block. It's just a simple module, but I think it's quite cool and I wanted to see how making such a module worked exactly.
If you were to feature it, I'd really appreciate a link to my channel: https://www.youtube.com/channel/UC4sI6CY2QyxYB3bufl4xZ4A
